The water flows along the channel above, and falls into the water wheel. The wheel spins, and drives a mechanical speed governor (the thing with the balls in front of the year), and also drives the mechanics for the clock. The governor works using weights; the balls swing out as it spins quickly, which activates a brake that slows it down (which causes the balls to swing in again and release the brake).
